Injury Waiver *

By checking this box I, the parent/guardian of the above-named player/cheerleader, hereby give my approval to his/her participation in the Russell Junior Football League during the 2020 season. I FULLY UNDERSTAND THAT FOOTBALL AND CHEERLEADING ARE POTENTIALLY DANGEROUS SPORTING ACTIVITIES and I assume all risks incidental to the conduct of the sporting activities of the Russell Junior Football League and transportation to and from these activities. I do further hereby release, absolve from indemnity and hold blameless and harmless the Russell Junior Football League, said League’s organizers, sponsors and the volunteers working therein, any or all of them, in case of injury to my player/cheerleader. I hereby waive all claims against the Russell Junior Football League, said League’s organizers, sponsors and the volunteers working therein, any or all of them, arising in any way from any injury suffered by my player/cheerleader. I likewise release from responsibility any person transporting my player/cheerleader to and from the activities of the Russell Junior Football League in regard to any and all claims arising in any way from any injury suffered by my player/cheerleader. I will furnish a birth certificate for the above player/cheerleader upon request of the Russell Junior Football League. I the parent/guardian of the above-named player/cheerleader, in case of injury to my player/cheerleader in my absence, hereby authorize any hospital and/or attending physician to give any emergency medical attention or treatment deemed medically beneficial to my player/cheerleader.
